Please provide your makeup availability (select all that apply) I can extend my lessons in the next few weeks to makeup for this lesson. For example, a 30 minute lesson will be made up by having two 45 minute lessons.

I can makeup the lesson another day this week. Please fill in the time range (ex. 3:00-7:00pm) on the days that you are available for lessons. Please try to give more than one day and time if possible. You can leave the forms blank when they do not apply.

I am available to makeup the lesson in coming weeks. Below is our availability (ex. 9/20, 3:00-7:00pm OR Weds, 3:00-7:00pm)

* If the cancellation is not made up, one cancellation allowance will be applied.
